SIGNING ON FORMS

Please could all clubs make sure that all forms are completed correctly before submitting them to be signed on.
This means that all the sections must be completed including the players date of birth, the form must be signed by the club Secretary the COUNTERFOIL must be completed and you must include a stamped addressed envelope for the forms to be returned to you.
IF any information is missing from your forms then they will be returned to you and the player will NOT be signed on until the form is returned completed correctly and the player will be ineligible.
Also please ensure that players forms that were signed on a Saturday are received by the Registrations team by Wednesday at the latest!!! Again those that are not will be ineligible for the game that they played in.
The Form must be the ORIGINAL.
